Ontario college requirements vary by institution and program. This checklist helps you organize; it does not replace the college’s official admissions page or instructions.
Personal and application information
- Legal name exactly as it appears on the required identification
- Current address, email, and telephone number
- Previous names, where requested
- Program name, campus, delivery format, and intake
- Ontario college or institution application number
- Student numbers used when requesting transcripts
Academic records
- Secondary-school transcript and graduation evidence, if required
- Post-secondary transcripts for every institution attended
- Evidence of prerequisite courses and grades
- Official documents sent through the method the college specifies
- Certified translations when documents are not in the accepted language
- Credential assessment only when the institution specifically requires it
Ontario Colleges states that colleges use transcripts to assess grades and prerequisites. The Registrar may request additional supporting documents depending on the program.
Program-specific supporting documents
Depending on the program, applicants may be asked for language-test results, portfolios, questionnaires, résumés, references, certifications, or proof connected to a placement. Health and community-service programs may have requirements that arise after admission but before placement.

- Portfolio, audition, questionnaire, or supplemental application

- Program-specific certifications
- Placement-related health, screening, CPR, or first-aid documents when requested
File-quality check
- Scan every page clearly and in colour when colour is relevant.
- Make sure names, dates, grades, seals, signatures, and page edges are visible.
- Use the file type and maximum size specified by the application system.
- Keep separate documents separate unless the instructions say to combine them.
- Name files consistently, such as LastName_FirstName_Transcript.pdf.
- Open every final file before uploading it.
